  • Date received, inspected, cleaned, and tested
  • Visible defects or contamination found
  • Repairs or refinishing performed
  • Test method and result
  • Pass, fail, repair, or quarantine status
  • Next scheduled inspection or testing date

When testing follows a failed inspection or unusual event, keep the tool, original report, and photographs available so the technician can compare the current condition with the concern that triggered the evaluation.
